Person-centred dementia support in the community

This project is developing an evidence-based framework for person-centred care for people living with dementia in their own homes, along with an organisational self-assessment tool to assist community care providers evaluate their systems, processes & structures to provide better support to people living with dementia.

The project was a joint initiative of Dementia Australia Vic and the National Quality Dementia Care Initiative which subsequently received funding from the Department of Health and Ageing: Encouraging Best Practice in Aged Care (EBPAC) program.

It is part-way through piloting a second version of the organisational self-assessment tool, and will develop an on-line version before rolling it out, with workshops and training through Dementia Australia organisations around Australia, to all 500+ community aged care providers in Australia.
